The Local Board services leadership team is committed to providing high quality, inspiring leadership. As part of this commitment we are seeking your feedback about how we are doing.

This 360-degree survey is part of a development programme we will be attending. The survey is built around a leadership model that was designed for Local Boards specifically, reflecting our unique co-governance environment - it describes the behaviours we want to see our leaders excelling at. You have been asked to complete this survey because we believe you will bring valuable insight to this person around what they are doing well and the opportunities for improvement.

Please read each of the statements carefully and make an assessment of the person's performance by providing honest, constructive responses on the form provided. There are no right or wrong answers, you have been chosen as a feedback respondent because this person is interested in what you have to say. You will have the opportunity to rate the person's performance using a 6-point scale - please try not to use only one part of the scale - e.g. all 5s.
