Introduction
As part of our leadership promise and our commitment to achieving our vision of

", we want to understand how we are performing, what we are doing well and what we can be doing differently.
On the following pages there are statements describing critical leadership behaviours and values. These describe our aspiration to leave people feeling cared for, to foster trust and collaboration, to be humble and genuine, and create an environment where people can learn and innovate.
Please read each of the statements carefully and make an honest assessment of the person's performance. There are no right or wrong answers, you have been chosen as a respondent because this person really values your feedback. You will have the opportunity to rate the person's performance using a 6 point scale - please try to avoid using one part of the scale i.e. all 5's. Instead try to use the scale to differentiate the things the person does well and where they can improve. You will also be asked for written feedback, we know this takes a little longer so thank you for the investment you are making - people always find this feedback most revealing and helpful.
